Required Documentation & Age Requirements

IMPORTANT NOTICE:

Coming prepared with the correct documentation ensures your appointment runs smoothly and prevents disappointment. We strictly follow Wisconsin state regulations and APP (Association of Professional Piercers) guidelines to ensure your safety. Without proper documentation, we will be unable to perform your piercing.

All minors must be accompanied by a parent or legal guardian with valid ID AND must bring their birth certificate.

Documentation Checklist for Minors

Before leaving home, make sure you have:

  1. Parent or legal guardian physically present (must stay for entire appointment)

  2. Parent/guardian's valid state-issued photo ID

  3. Minor's birth certificate (original or certified copy)

That's it! The minor does not need their own photo ID.

Acceptable Forms of Identification

    • State-Issued Driver's License (current and not expired)

    • State-Issued ID Card (current and not expired)

    • Valid Passport (U.S. or International)

    • Birth Certificate (original or certified copy) - REQUIRED

    Important: Minors do not need to bring their own photo ID. We require the parent/guardian's valid photo ID and the minor's birth certificate to prove guardianship and age.

Additional Important Information

    • Eat a full meal 1-2 hours before your appointment

    • Stay hydrated throughout the day

    • Get a good night's sleep

    • Avoid alcohol and blood-thinning medications for 24 hours prior

    • Come with clean skin in the area to be pierced

    • Double-check you have all required documentation

    • We'll review all paperwork and consent forms

    • Josh will discuss jewelry options and placement with you

    • All equipment is single-use and sterilized

    • Complete aftercare instructions will be provided

    • We use only high-quality implant-grade materials

    • You do not have the required documentation

    • Minors arrive without parent/guardian present

    • Minors arrive without their birth certificate

    • You appear to be intoxicated or under the influence

    • You are pregnant (for certain piercings)

    • You have active skin infections in the area to be pierced

Appointment Policies

  • DEPOSITS ARE NON-REFUNDABLE

  • Rescheduling is available as long as notice is given at least 24 hours in advance of your current scheduled appointment.

  • If you show up 10 minutes or later past your scheduled appointment time, there may not be enough time to complete your appointment before the next appointment and we may need to reschedule.

Requirements Per Piercing Style

  • Age Requirement: No minimum age in Wisconsin, but our studio minimum is 4 years old

    Required Documentation:

    • Parent/guardian's valid state-issued photo ID (driver's license, state ID, or passport)

    • Child's birth certificate (to prove guardianship)

    • Parent/guardian must be present during entire appointment

    Special Note: For children under 4 years of age, special arrangements must be made in advance. Please contact us at (715) 650-7131 to discuss availability.

  • Includes: Helix, Forward Helix, Tragus, Conch, Daith, Rook, Industrial, and other cartilage piercings

    Ages 16-17 (Parent/Legal Guardian MUST Be Present)

    Required Documentation:

    • Parent/guardian's valid state-issued photo ID

    • Minor's birth certificate (original or certified copy)

    • Parent/guardian must remain present to sign consent forms

    Ages 18+

    Required Documentation:

    • Valid state-issued photo ID (driver's license, state ID, or passport)

  • Includes: Nostril, Septum, Eyebrow, Lip, Labret, Monroe, Medusa, Philtrum, and other facial piercings

    Ages 16-17 (Parent/Legal Guardian MUST Be Present)

    Required Documentation:

    • Parent/guardian's valid state-issued photo ID

    • Minor's birth certificate (original or certified copy)

    • Parent/guardian must remain present to sign consent forms

    Ages 18+

    Required Documentation:

    • Valid state-issued photo ID (driver's license, state ID, or passport)

  • Includes: Navel, Surface piercings, Dermals, and other body piercings

    Ages 16-17 (Parent/Legal Guardian MUST Be Present)

    Required Documentation:

    • Parent/guardian's valid state-issued photo ID

    • Minor's birth certificate (original or certified copy)

    • Parent/guardian must remain present to sign consent forms

    Ages 18+

    Required Documentation:

    • Valid state-issued photo ID (driver's license, state ID, or passport)

  • ge Requirement: 18+ ONLY - NO EXCEPTIONS

    We do NOT perform these piercings on anyone under 18, regardless of parental consent.

    Required Documentation:

    • Valid state-issued photo ID (driver's license, state ID, or passport)

    • Must be able to prove you are 18 years of age or older
